It is important that we have the participant's best contact information in case we need to reach them (especially outside of their business hours) for clarification and/or any last minute changes or information they may need. If other information is given we can not be responsible for lack of communication or expenses incurred as a result.

For a group rate to apply you must register all participants on the same form.

Additional Requirements only for COTAs, OTAs, CODAs, SAs, PTAs, or students in training. LICENSING STANDARDS - All students and assistants must follow individual licensing/training standards for their State/Country and their professional degree’s supervision standards. When there is a requirement for students and/or assistants to practice only under professional supervision, the SOS Approach to Feeding program requires those SLP-A’s, COTA’s or discipline equivalent to have a Supervisor who themselves have attended a SOS Training Conference. If your Supervisor has previously attended the SOS Approach to Feeding and you are registering to attend an SOS Training Conference as an assistant or student, please have your supervisor send an email stating that they will provide supervision to you as required by your State/County’s standards while practicing the SOS Approach to Feeding program, along with a copy of their SOS Certificate of Attendance. If your supervisor is planning to attend an SOS Conference with you, please have your supervisor send an email stating that they will be attending the SOS conference with you and will provide supervision to you while practicing the SOS Approach to Feeding IF this is required by your State, Country or professional degree’s standards. If you are uncertain of the licensing standards please have a supervisor of the student/assistant send an email to cover this requirement. Please email this communication to [email protected]. *

A Student is registering to participate. A COTA, OTA, CODA, SA or PTA is registering to participate. There are no students or assistants registering.

I agree to the Attendance Policy: I agree that I am attending a Live Conference in Portland, Maine on August 11-14, 2022. To ensure Learners successfully achieve the Learning Outcomes listed in the Learning Outcomes Section of this course description and to satisfy the completion requirements for the SOS Approach to Feeding conference and continuing education credits, all attendees are expected to attend the course for the entire duration of each day (7:30 am – 5:00 pm, Days 1-3 and 7:30 am – 3:00 pm, Day 4). Additionally, all attendees are required to complete a test at the end of each day and an evaluation form at the end of the conference in order to receive their certificate of attendance. Leaving early or missing time during the conference results in missing crucial information needed to correctly use the SOS Approach to Feeding program. Please make any travel arrangements to ensure that you do not arrive late or leave the conference early. Partial credit will not be given for this course. I agree to the Cancellation Policy: I agree that I have read the Cancellation Policy in the Information Section under Policies completely and agree to the policy in full. I agree to the Payment Policy: All registration payments must be made in full or have an approved payment arrangement/issued purchase order with Coastal Education Services prior to the start of the conference in order to participate. I agree as the Attendee that by registering and attending the SOS Approach to Feeding conference, both the Host organization and Toomey & Associates, Inc. has access and permission to contact me (or the attendee) by phone, email or mail. This information will not be shared or sold to any outside organization, but will provide the attendee with access to information and resources pertinent to the SOS Approach to Feeding conference and program. At any time, I can opt out or unsubscribe by contacting either organization. Furthermore, if I am acting as an agent for the purpose of registering an attendee, I am agreeing to notify the attendee(s) of this agreement, and am agreeing on their behalf, to receiving any and all communications by the host and Toomey & Associates, Inc. I agree that by checking this box I am verifying that I am the person registering for the course/activity and that by checking this box it will act as my online signature. If we have more than one participant on our registration form, I accept that we will each be contacted using the information given in this form to also agree to these Terms and Agreements. I agree that I will inform Coastal Education Services of a substitution, if necessary, by following the guidelines in the Cancellation Policy found in the Policies Section of the Information Page of this conference.
